SMN is headquartered in Raleigh North Carolina, and is part of the SembraCare group of affiliated companies. SMN is a messenger network, designed to enable certain North Carolina providers of Long Term Services and Supports (LTSS), Home Care Agencies, Adult Care Homes and Family Care Homes, to contract with the five Prepaid Health Plans (PHPs) who have been awarded contracts with NC Medicaid to implement Managed Care beginning in 2019.
SMN builds on the successful skill set and expertise of SembraCare, which has decades of experience serving NC Medicaid providers. The root word SEMBRA in our company name means “to plant a seed” and echoes our goal: to support providers with the services and tools needed to grow and prosper.
